Doing hangar missions and got the "follow slow ass titan" one. Followed it for 10 minutes (Hydra hover FTW), got the crate, returned to the hangar, and landed a bit too hard. Hydra exploded and the secondary explosion destroyed the crate. Yup, that was the last hangar mission for the day.

I mess up the Hydra hover often when trying to be precise. It slides around and often throws me into a tower or building or too close to my target when using the explosive cannons. I also keep holding it to descend, which cuts the engines causing a crash. For a bit of MnB, I hate how the Pegasus Hydra is better than any other aircraft for the hangar missions. It's fast, hover for picking up crates, explosive cannon for any enemies you can't lock on to, and for $200 to spawn it's cheaper than the $1000 insurance call for any personal aircraft you could use.

The following scenarios make me automatically kick myself before the job even starts:

1.  I select Random Heist and Prison Break comes up (just because of the glitch where Rashkovsky doesn't advance...I'm not even taking the risk of it happening)

2.  I join a random heist and a level 15 joins.  Nope, sorry bud, you might be a good player but odds are you're going to die continuously.

Arrows to Athens

When there's an enemy around the corner, but you don't know because you don't hear them and you don't see them on the radar, so when you walk past, they plug you with bullets and kill you in less than a femtosecond.

Running over Trolls. I'm lazy so I just use the Broomstick. It usually works no problem unless you activate the boost on a hit then it can ragdoll them pretty far and lead to some fails. Failed one because the guy in Sandy Shores got yeeted into a car a block away. Another time because the guy flew into the air like 100 feet and died from impact. I try to remember not to do the boost during that hit but I'm so used to doing it other times since it's funny watching them fly.

Missions with coronas where you break through the door to fight. If you're too close or quick, you can swap weapons before going in but it doesn't register so you go in with your old one. My problem is I like to use the Stone Hatchet inside. Nothing like busting into a Lost MC club and running up to the first guy and melee-ing him with a rifle only to get lit up from 6 directions because there's no rampage activated. Similarly at Tequila-La, I'll activate the Stone Hatchet upstairs and drop down to the bottom. Every so often the drop goes wrong and my guy faceplants or does a flip. The delay can be enough for the rampage to wear off and I'm similarly getting lit up from 6 directions.

Bank Contract. Forgetting to get out of the vehicle at the start and putting your weapon away. If you don't you 10-15 seconds of cop alert time which makes getting through 2 banks a little tougher. Also parking too close and forgetting to shoot the customers at the final banks. They'll run outside and block your car.

Forgetting to stock up on armor before logging off. It's rare I do but it sucks logging on and doing a mission only to realize you don't have armor.

In Vehicle Recovery for Ballas/Vagos. Forgetting to open the garage door before getting in the vehicle. This spawns in more enemies from all angles.

Nightclub crate source missions at the docks. In all the time I been playing I never once saw an NPC on my side until I noticed the security guard at some locations at the docks are actually shooting the gang members and not at me. They be putting in work too. Last night I kicked myself for shooting the security guard mistaking him for a gang member. Thank goodness it was only one shot and he survived.

There's a reason why I always park INSIDE the bank lobby when doing this job.
