Looking for a BLACK LABEL copy of Grand Theft Auto: London 1969 for the PlayStation, with the map, manual, and case

These are surprisingly rare. The only ones on eBay are either disc only, missing map, missing manual or other artwork slip, horribly destroyed jewel case, the later release that has “collectors edition on it”, OR they are brand new factory sealed, which is a lot more expensive. 
 

anyone come across the original, black label version, with everything that originally included with it, but not necessarily factory sealed, just in good condition?
