game freezes then crashes (mods and crash log)

the game will freeze for a second or two and crashes randomly with no errors popping up. sometimes it just crashes after 20-30 mins of playing, sometimes when entering a vehicle or a building, like the gym sometimes.

my game is running as administrator with compatibility mode for windows 98 / windows me. DEP (data execution prevention) is also turned on. here is a crash log:
