jimmycakes
There's something twitchy and light-weight about the aiming. Usually I'm good at leading a target or focusing on a point in games, in RDR2 even with different sensitivities and mouse smoothing it feels hard to control and fight enemies especially if they're on horseback or zigzagging. Xerukal
There's a built-in sway to aiming in this game, it was a problem on console too. It's not nearly as smooth as other games, but I believe part of it is by design. You kinda get used to it eventually.

Dryspace
I just want to point out that mouse smoothing does add latency. A small amount of smoothing may not be noticeable, but increasing the number of samples that are smoothed directly increases the lag. I don't know what kind of smoothing you're using, but it's at least possible that it could be making the aiming situation that much worse.
