My GTA v disc does not appear to be damaged, but it keeps restarting when I'm at the loading screen and going black at random times.

I had GTA for about 6 months and it was working fine until 2 months ago. It started out with the screen going black in story mode and me having to redirect myself to the home screen, where I'd restart the game, only to have the SAME thing happen again. Now, whenever I try to get online, it restarts and says "alert, your game has been restarted due to a damaged disc" although there is no damage besides one scratch on the outer edge of the disc. Please help me fix this bug! I'd greatly appreciate it.

Also, I forgot to add that I'm on Xbox 360.
