Conspiracy theory: the Stranger is Heidi's dad, or hubby/fiancé. When Dutch murdered her in cold blood, Marston shot that man in cold blood too. He represses that memory until the ghost returns in 1911. Even if that ghost is actually a fragment of his conscience. Like, to me, I agree with the idea that the guy was some kind of God or Satan like figure, and he was just making a point about how easily life gets thrown away, etc. Stuff like that. I'd be fine with being wrong, just so long as i see the guy again in RDR2. I never considered him being a ghost, but when Marston says "DAMN YOU!" and he replies "Many have..". makes me think he's supposed to be god or something. And why identify one girl in particular, out of the hundreds Dutch and John murdered over the years?
