For the very first time during the promotional Grand Theft Auto IV campaign in the nearing months to its release for the console systems back in the beginning of 2008. It was also a part of other giveaways and similar events like this on in particular:

Status and Availability:

Hard to say. Number is definitely unknown, but might be to some extent limited. Indeed, there were a couple of occasions when the aforementioned item was introduced to the public and community as a bonus goody. Searching through the net, some stories suggest that the full boxes of it were spotted. Anyway, it can be found from time to time on e-commerce sites, but not as a regular guest.
