After the assassination of his father, Vito Manzoni, the Don of a small mafia outfit in Sicily, his son, Giovanni Manzoni, immigrated to America to the city of Los Santos seeking retribution and for the chance to start a new life. But after dipping a pinky back into this criminal underworld pond something reached out and dragged him back into its depths. Filled with hatred and with greed in his eyes Giovanni created his own "Cosa Nostra". After his arrival, Giovanni stumbled across a close friend from the old country, Tommy Roselli.

With the aid of Tommy, Giovanni fought hard to take over the Port of Los Santos "The busiest container port in the USA" with approximately $236 billion worth of cargo coming through it every year. Siphoning off and selling incoming cargo the friends made a fortune. Their efforts were not going unnoticed, however. Local thugs also wanted a piece of the cake fighting tooth and nail with the friends. Eventually, the friends were beaten and forced into a corner. From this Giovanni decided to kick his own "Cosa Nostra" into full gear. Giovanni knighted himself Don of the Manzoni Crime Family and made his close friend, Tommy, his Consigliere. Now, Giovanni and Tommy are in search of members for their Mafia outfit.
