Request for An Award or certificate!

Theme : Hi, I'm Ken! I'm in a need of an award type, maybe like a banner but in a 450*200 or 500*210 dimensions. The theme is featured musician of the month. Actually, it's all about music and guitar. So, a guitar, probably the one with fire or something glittery.

 

The central text should be "Featured Musician of the Month" and the rest should be, like the footer : Congratulations for your excellence in music. The middle space where the name of the person is to be blank, probably, something which can be edited. Yes, also, above the "Featured Musician of the month" I need this text : "Guitar'asm", probably in bold, underlined, just so that it can be spot out from others.

 

The colors, something black and firey type, like which gives a very brightening look, but not too bright. I hope you all understand.
P.S : A picture of guitar legend, probably, Jimmy Page would be desirable.
