I think it's more believable that they'll just add a new island that you can fly to, maybe with a loading screen in between.

I doubt that, if they did something like that they would have to remake the cutscenes and endings to heists such as Series A, Humane Labs and Prison Break. If anything it would be much, much smaller and located just off the Palomino Highlands as the Prison Break cutscene is pointed upwards so it would be largely unaffected.

Actually from what I know of, the cutscenes are rendered in real-time in the game so they would show the new map automatically. However I don't think it really makes sense to make such a huge change to the existing map and acting as if it's always been there. Also things like the lighthouse would just disappear. It's probably going to be a separate island in a separate instance if anything.

Trevor dives off the cliff at the lighthouse into the sea, and then swims to shore. That entire cutscene would be rendered useless as even if it was as simple as you say, Trevor would then be diving onto a rock or a sanddune or something of the kind instead. Which would probably look pretty ridiculous, plus he would be more likely to get hit by the bullets that are shot to him then if he is underwater...
