Need help w/ low framerates on GTX 760

So I've spent the last day running benchmark tests and attempting to improve my framerates, but I am running into some serious issues.

First of all, I'm running an Nvidia GTX 760 with an EVGA x58 FTW3 to go along with a LGA1366 i7 chipset (yes I know the mobo is 2.0PCI), 12GB of ram and a 1TB HDD.

I am getting a constant 12 FPS in the game currently, even with Low settings. If I change the graphics to Ultra, there is still no noticeable change in my framerates.

I have checked my cores, and they are all running. I've been using MSI afterburner to overclock the card to hopefully compensate for a little, but to no avail.

I am running Nvidia's current driver (which was released solely for this game it seems).

I have tried every fix I could find online (including setting affinity to high, so on and so forth) and it seems that there isn't much left for me to do.

Could someone PLEASE offer me some help into figuring out why my framerate is so low? I've seen people running the same rig as me and pulling 60 fps easy.

Any help would be greatly appreciated.

What massively improved my frames was changing V-Sync from "Half" (Default for me) to either "On", or "Off". Gave me 15-20+ frames, in total I now get 45-50. I'm running a r7 200 series though

can u post a log of the msi afterburner gpu usage and that? sounds like card does not go up to 3d clockspeeds and is on half speed or something.. best is to uninstall drivers and install fresh again. they could be corrupt maybe.

That's the point of "Half" Vsync >.< 30 fps

your title says 760 but description says 960...

i got two 760's. with one running i can confirm with i7 + 16gb ram... using nvidia optimised settings @ 1080, its stable 60 fps in city, and average 30 fps on heavy foliage with alot of "action" around the foliage.

i've noticed that the location of the game can affect gameplay. the emptier the installed drive, the better. SSD have an upper and. also use nvidia optimised settings.

maybe look into the BIOS version!?!

My apologies. It is a GTX 760. It was late and I was running on very little sleep. I have my power limit set at 115% on MSI Afterburner and i'm only getting about 45% of that power.

My HDD is nearly full with about 100GB of storage left, don't know if that could be an issue.

I flashed the BIOS and updated it's drivers as well.

Here is my log idle.

And while running GTA5

My bad, I'm completely new to PC gaming, so I'm still experimenting everything possible.
