I've been finding the same issue - it only ever happens during Heists, and it appears to swap the facial structure with that of another player. Heading to SP and back to MP fixes it, albeit temporarily.

This happened to me during a heist yesterday. I turned into a tubby faced black guy. I would have lived with it had I had to, but a 2 minute search of the Rockstar support forum told me to go to...

 

single player -> start -> online -> choose character

 

...and it was fixed. They've acknowledged the issue and that resolution worked for me.
