GTA V Keeps Restarting (xbox 360)

Since a week now my gta v wont load
by ''loading story mode'' the game crashes and restart (NOT THE XBOX)

The loading screen where my game crashes (shown down below)

I Tried to
-reinstal gta by removing the part files and re instal it by disc 1
-move files to usb and format hardisk and put them back on
-cleaning the disc (2x my sad the disk was unreadable i put it a second time back in without cleaning it and it runs(stil the same problem came))

IDK what to do?

help will be appreciated
