Its likely just your connection. Whenever my connection craps out the city disappears, but everything is fine in singleplayer.

This used to be a serious problem with my old PS3, it was extremely slow at reading disks and in general. My new one runs better, it sometimes lags a bit but nothing bothersome.

I find this happens nearing every update to be worse, im assuming there are tests and such going on that glitch the game out. Or what everyone else has said

PowderFreshness

So far this afternoon, I have frozen twice, spawned underground, and spawned invisibly with the ability to shoot but not move.

Did you install the play disc? That's probably why if you did. Rockstar even stated that it's recommended to not install it when GTA 5 first launched.
