It's an external mod/tool (on your computer).

 

You transfer the carsave (via USB) onto your computer and edit it there. (I think, don't hold me on that)

 

Then you transfer it via USB back onto your console (there's different save editors based on your console), then do some mod magic and bring it into SP.

 

SP2MP and you're done

If you can do something that requires a computer or program of some sort, it's a mod/hack.

The nomenclature doesn't really matter though.
