I believe you can only modify your parents once and thus your physical appearance after you create your character, except for hair of course, can always change that. So you'd have to make a completely new character.

Sometimes you have the possibility to change your character's appearance. It's random though.

This. I made a random character during the whole not getting past the first race who looks like a baby face ludacris. Well turned out I got past the first race, then later on I got the option to change my character one last time and decided to keep the look. Now I kinda regret it.. So maybe we should just switch characters?
