Crashing at loading story screen

I have had the game since launch and it has worked fine but earlier today I turned my xbox on to find that i was receiving the storage system error forcing it to restart and not load.

 

I read the rockstar page that said to delete all files and re-install both disks this leaves me with the exact same issue. so at the moment i cant even play the game online or offline. Another thing I tried was installing the game onto my hardrive which crashes at 42% saying that the disk needs cleaning.

 

I am just wondering whether its worth replacing the disk or whether its an xbox issue. I've seen the pages about installing one of the disks to a USB but is this necessary?
