You're right. When you die, you shouldn't pay hospital bills, and - by association - you shouldn't respawn either.

You respawn randomly that way someone in free roam cant sit at one of the hospitals and tell their buddy to keep killing anyone they can find, while they sit there and take out the people that die and spawn at the hospital. Common sense
