I think they used to be able to find them in the software files but there may be more red tape nowadays. Sometimes the company will leak certain codes to gameplay guides, magazines, etc. in order to generate some revenue. Most of the time the cheats are given to the authors of the gameplay guides and / or other magazines. Also if it's doing random combinations how does it know how many of the previous combinations actually enabled the cheat? (assuming it can even know when a cheat has been found).
