The bottom of this tattoo is visible under his sleeve in the same scene that Michael is quite clearly alive. It is a permanent tattoo. Maybe Michael presumed dead when he went into Witness Protection is Trevor got a tattoo to commemorate him before later finding that he is still alive and get back together? Also it further supports the Michael-Trevor Brother theory, they must have been really close for Trevor to get such an elaborate Tattoo for Michael. This tattoo refers to the death of someone called Michael, that's pretty obvious. What I don't know is what is it doing there.
