Graphical Error

I'm having this graphical error in my game play. I do have some mods like LCPD:FR, some add-ons for that and some cars also. I was hoping someone could assist me cuz this is very irritating. During the glitches I can play normally, just the visuals are jacked so I can't see sh*t.

 

 

Computer: (Not the best, I know.)

Processor: 2140 @ 1.60GHz

RAM: 3 GB

OP: Windows 32-Bit

Graphics Card: NVIDIA Factory Overclocked EVGA GeForce GTX 460

 

I run GTA IV on all low settings 1440X900 resolution VSync is On

[CCPD]

I'm pretty sure that with the amount of mods, which are almost always very demanding on virtual memory (high resolution textures, high polygon count), you system finds it hard to cope. As a result it takes something off to compensate and keep it playable.
