GTA:SA was good for what it was, but I think now that R* has shown us what they can do with a realistic setting and dynamic, GTA should keep to being realistic, but not over the top realism either. Of Course) Invisible Barriers (Ask why? Play Mercenaries 1 or 2) Use of Drugs outside of missions and cuts (I like the getting drunk in GTA IV but thats about it) Traffic Felonies (See Traffic Felony mod for GTA IV) If a GTA III era person returns in any shape or form. (It's dead and over we have no need to hear their story again.) If your guy dies and you take over someone else of kin. If the MP ranking system is like GTA IV! In Salt, there's a scene where she escapes from her pursuers by dropping off an overpass onto the roof of a truck, and as they chase her she has to leap over to another truck -- all at freeway speed -- to get away. It's ridiculous and pretty much impossible, but in an action movie context it still kind of works. But if she had leapt all over the freeway from vehicle to vehicle, crushing them like the Matrix twins, it would have ruined the movie completely. That's the line I want V to stay very close to.
