RanxeroX
Sounds like you have the proper tools for the job but what you need next are the skills (no offense!) if you want to create it yourself... The quickest way to do what you're after is to paste an appropriately sized image of the face portion over top of the old one then manually draw the other details. But do this you need some drawing skills to envision how half the head should be drawn in 2d so that it wraps onto a 3d model and then be able to flesh it out onscreen. I've done a few skins for personal use this way but maybe there are alternatives out there that would make the job easier. I can think of a couple off the top... First, there might be a piece of software that can do what you want - don't ask me what that might be. Second, I'm pretty sure it could be done in a much more complex way by using a 3d modelling program's ability to flatten any textures applied to a model for the purpose of generating a single UV map. It would involve creating the model, applying separate textures to each part of the head in your case, then selecting an option to generate the UV map texture. Once you have that it's just a matter of sizing and placing it into a combined graphic like the sample you show.
