Here are some of my ideas

 

Put it in Paris, France

 

Have a customizable protagonist at the beginning like most RPG's and have a choice between male and female (like in IV's multiplayer)

 

More Mission Impossible type missions (if it's gonna be in europe it's gotta have style!!)

 

Less exotic cars more Common cars, and make a few hundred types of cars (for real R* if you want realism spend time on it)

 

More interiors (see above parenthesis)

 

(people are gonna hate me) gay dating (they may have hinted at it with french tom (no pun intended) so i want it for realism and personality)

 

wwwaaaayyy more brands of clothing (even more than san andreas had!!!)

 

EXPLOSIVES!!!!(please rockstar!!!! we love sticky bombs)

 

More resteraunts (i don't wanna live my life eating chicken and burgers)

 

Seasons (and yes n00bs i know it's not the sims but it should still be fun)

 

Bring back car custimization (i loved it, i used to spend hours finding my favorite car and then custimising and role playing driving to work in it)

 

Talk Radio (i know it's there already but bring back the paranormal show from SA it was my favorite, hell bring back all of WCTR)

 

Put in real myths (make them rare but still there, kind of like you did with leatherface)

 

Also put lots of subliminal messages (like you could of with the rocks in san andreas)

 

Custom Radio station (for the console, just let us pick from the games playlist like we could in MC LA but make it a radio station instead)
