If you get the scrubs then die and replay the mission you will already have the scrubs and so they won't appear again because you already have them. I didn't even realize you could still use it until I saw my brother wearing it the other day. The black balaclava is the ski mask from "Three Leaf Clover". Some people just word it differently.
