It makes their likeness and respect go down, that's about it.

 

It will also remove their special abilities if you ignore them long enough.

Just turn on sleep mode on your phone, this blocks all calls and doesn't effect respect stats.

A better way to do this instead of putting your phone into sleep mode and missing out on potentially important phone calls/text messages is to accept their invitation, then once you hang up, just call them right back and it will cancel the event. thus, no negative thumbs down.

 

Obviously the more time you go without doing something with a friend you will randomly get a thumbs down. I kept saying no to the requests for a long time before i realized i could cancel and not get a negative reaction, as well as before i knew i needed the friendship to go up. Mine got down to like 20% with the friends.
