shenaniganz
I learned this today, I don't know if it's common knowledge but I don't see any FIX's?! I couldn't play GTA4 without taking out the harddrive. Take the disc out, restart console. Main dashboard, slide over to the right, go into the memory settings tab. When selecting whichever memory source (harddrive), click the one you use, obviously. Hit Y to enter somethingsomething settings. It'll say format, and something else. Go ahead and format it. Just kidding, don't. Just hit XX, LB, RB, XX. Another screen will pop up. If you don't get it at first, do it again. I did it like 5 times at different speeds before it worked, I was starting to not believing the person telling me to do this, I've never seen this screen before. It's either yes or no, click yes. That's it.

xaxsis
The ABOVE didnt work for me, however heres how I fixed mine: Preamble: this works for most cds/dvds that are "damaged" (even if theres no visible scratches). Im assuming no one has any gapping scratch, where of course this will not fix it. Step1- Hold the disk under running tap water. Step2- Use toothpaste (just the standard type, dont use any fancy gel types) and gently spread it over the disk. Step3- Rinse under water to remove the toothpaste Repeat the above again Final step: allow the disk to be completely dry before putting it back in the 360 tray. Use a disk friendly cloth to speed this up. Make sure its dry and no toothpaste is on it or your likely to screw over your drive. You're now ready to play GTAIV without any clean disk errors. Clean the cache anyway, however once the DLC comes, it will wipe that off, and download it again. Obviously to most the above will sound ridiculous, however it does work, search online if you need more reliable proof. Anyway goodluck, enjoy your minty fresh error free copy of GTA IV
