To gain extra health you need to complete the paramedic missions. To do these get get an Abulance and hit 2 to trigger the mission. You need to complete them to level 12 in order to get extra health.

 

In the picutre your showing the health looks about double the size it should when the extra is aded on. In my opinion it looks like it might be mod but I could be wrong, but anyhow if you want extra health just do the paramedic side missions.

yeah i've seen that a few times and wondered how they get that.

 

i've also completed the paramedic missions and my health bar didnt increase past the lower left corner of what weapon your carrying is displayed.

Health bar also increases during the game when CJ walks and training a lot. When I reached Las Venturas, I had 150% of health. And I did complete Paramedic cause I wanted to have 100%.

It's a mod, I saw someone posting a proper screenshot of it. But AFAIK you can't get this much health in normal game. Ambulance gives you just enough health to fill the gap under the weapon icon.

If you exercise a lot (walk, run, cycle, not necessarily gym exercise) CJ's max health will go up.
