No you dont see bullet holes the area you shoot becomes red, this applies to Niko aswell, if your shot in the leg your leg is covered in blood

Yes some pedestrians do carry guns, most of them being the least expected to carry them. I was watching a live feed of GTAIV yesterday and the guy playing drove a car through the burger shot knocking over a fat man, as a result of driving through the store he niko hit a table and shot out the front windshield landing kinda crippled on the floor. The fat man who got knocked over got up and started shooting niko as he was down for a classic death by a civilian
