Lester was never shown until you get to the first heist setup. Did any of the guys in the prologue look as fat as Lester? No.




You can gain a lot of weight in 9 years

My best guess for Lester based on what we've seen: Arthritis.  Probably Ankylosing Spondylitis, judging by it how it seems to only affect his back. He's also around the age where it would make you walk with a cane/hunch and use a wheelchair if not treated aggressively.

Apparently, according to GTA V Wiki thing, it is slowly attacking his motor control (bodily movement), so at a guess I would say, probably MS (Multiple Sclerosis)?
