Seemingly every interesting "hidden" place in GTA IV is part of a mission at aome point.

Are there any truly hidden places worth discovering? (No basic apartments, laundromats, etc.) Something that's actually unique...

GTA4 Liberty City Map - IGN would be interesting think for you to look. There are many interiors listed with map. Can't tell though is there any interesting interiors not used in mission, but hotel suite (Entered via helicopter) and hotel lobby on the south side of Middle Park are first think to come to my mind

Do you mean the "Majestic", that a mission takes place in? Your target is some guy on the roof, during a party?

In original IV? I don't remeber that kind of mission, though have completed whole game.

You could check the Interior of the Happiness island
statue.
Not the original entry but the secret entry with a helicopter.
Fly a heli to the top balcony, jump out and look for the sign,
"No hidden content this way"
And then enter the door,Finally climb the ladder to the top to see the unique thing you wanted.

In multiplayer you can go in the hardware store on Dillon Street that Roman gambles at.
it consists of basically a able and 5 plastic chairs, not very interesting but a cool place to hide in blip-off games.

Yes, the one in It's Your Call is accessible in multiplayer.
