Have you updated the game?
What settings are you running the game at?
Do you have any mods installed?

Can you give us a little idea on FPS you are experiencing?

Reason being that we may find that the lag you are encountering is normal for a system with your spec's

Agreed. Could you try lowering the sliders in the video settings or maybe reduce some of the other settings as well? Is this the first time you're experiencing lag on this system, or a better way to put it has the game run smoothly before? If not I suggest you at-least upgrade to a Quad Core CPU since IV needs a good processor to run it and the Intel Core 2 duo E7500 @ 2.93Ghz is relatively weak.

Since the game was running smoothly before some of the settings had to be increased then in this case. I think you may have done it by accident. Have you tried any of the suggestions stated above because we need to know that.

your videocard wasn't enough for gta iv to begin with

and now it may have reached its thermal threshold, which means it overheats even sooner, which would account for lagging

agreed GTA needs gaming hardware and you have a multimedia card . looks like that you are lacking of Ram too . 2 Gig is really short
